COVID-19 - Waiver This waiver & declaration must be completed prior to completion of your registration and before participating in any activity. The novel coronavirus, COVID-19 has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is known to spread mainly by contact from person to person. Consequently, local, provincial and federal governmental authorities recommend various measures and prohibit a variety of behaviors, in order to reduce the spread of the virus. (Quick Stick Lacrosse) and its members commit themselves to comply with the requirements and recommendations of Ontario National, Provincial and local Public health and other governmental authorities, and to put in place and adopt all necessary measures to that effect. However, (Quick Stick Lacrosse) cannot guarantee that you (or your child, if participant is a minor/ or the person you are the tutor or legal guardian of) will not become infected with COVID-19. Further, attending the Activities could increase your (or your child, if participant is a minor/ or the person you are the tutor or legal guardian of) risk of contracting COVID-19, despite all preventative measures put in place. 1) I acknowledge the highly contagious nature of COVID-19 and I voluntarily assume the risk that I (or my child, if participant is a minor/ or the person I am the tutor or legal guardian of) could be exposed or infected by COVID-19 by participating in Quick Stick Lacrosse activities. Being exposed or infected by COVID-19 may particularly lead to injuries, diseases or other illnesses. 2) I declare that I (or my child, if participant is a minor/ or the person I am the tutor or legal guardian of) am participating voluntarily in (Quick Stick Lacrosse)’s activities. 3) I (or my child, if participant is a minor/ or the person I am the tutor or legal guardian of) agree to the requirements and recommendations of National, Provincial and local Public health and other governmental authorities and to those special safety regulations put in place by (Quick Stick Lacrosse) as it pertains the Covid-19 Coronavirus and to adopt all necessary measures to those effects. 4) I (or my child, if participant is a minor/ or the person I am the tutor or legal guardian of) agree that, by filling out and signing this waiver and agreeing to the terms and conditions set out in it, I am giving up my legal rights to sue (Quick Stick Lacrosse) and its coaches, officers, and directors, in the event that I (or my child, if participant is a minor/ or the person I am the tutor or legal guardian of), contracts the Covid-19 Coronavirus. This document will remain in effect until Quick Stick Lacrosse, as per the direction of the national, provincial and local government and health officials, determines that the acknowledgments in this declaration are no longer required.

By signing this waiver I, or if I am under 18 years of age my legal guardian, expressly agree and promise to accept and assume all risks existing with the activities operated by Royal City Ball Hockey Club (the “Club”). My participation in the Clubs activities is purely voluntary and I elect to participate in spite of the risks associated with such activities. I understand I have the right to withdraw from any activity or situation I feel uncomfortable with. I now freely accept and assume full responsibility for all risks to me in connection with my participation in Club activities.
I waive all legal rights to bring any and all claims I or my successor may have against the Club with any loss, injury, damage or expense of any kind that I may in any way suffer, incur or experience as a result of or in connection with my participation. I hereby release the Club and their supervisors, officials, sponsors, organizers and corporate owners of the premises from any and all liability for any complaints, demands, claims, actions, suits, judgments and orders for any and all losses, injuries, damages or expenses I may suffer, incur or experience in connection with my participation. I agree to indemnify the Club for and hold it harmless from any and all losses, injuries, damages and expenses of any kind that the Club may suffer, incur or experience and for any and all complaints, demands, claims, actions, suits, judgments and orders for any and all losses, injuries, damages or expenses of any kind anyone else may suffer, incur or experience in connection with my participation. From time to time the Club publishes photographs taken during Club events which may be used for editorial trade, advertising and/or any purpose and in any manner and medium. By signing this waiver, I further grant, released parties the right to photograph and/or take videos of me or my child participating in Club activities
